Although different information is required for different destinations, Trip Forms can ask for the following: payment information, emergency contact information, proof of medical evacuation insurance, trip insurance, past medical history, current medications, physical limitations, dietary preferences, passport information, arrival information, departure information, and a Release of Claims.

We may use personal information to provide the services you’ve requested, including services that display customized Trip Forms. We may also use personal information for auditing, managing and enhancing customer data, lead generation, and research and analysis to operate and improve Expedition Wild.
